By the late 1970s / early 1980s, “Scantron-type” bubble sheets had gained wide adoption across K–12 schools and universities in the U.S.
The fact that your sheet uses the “classic blue/green bubble-sheet” style — and not a newer black-and-white or vertically oriented modern form — suggests it predates more recent form revisions (which started appearing more broadly around the 2010s–2020s).
Such sheets remained in heavy use through the 1990s and 2000s, but began to decline more recently as testing moved towards digital formats.
